Supportive Housing Team Leader in Buffalo, NY

Supportive Housing Team Leader: Provides person-centered services to an assigned caseload of adult consumers. Services include assessments and evaluations, care planning and coordination, outreach and crisis intervention, linkages to services, and procurement of needed resources. Performs recordkeeping/documentation, statistical, training, and quality assurance activities. Serves as a mentor and coach to case managers and helps CHP Coordinator as needed.

Minimum Requirements: High School Diploma and five (5) years full-time work experience working in the Human Service Field, plus 1 year’s paid experience in a direct service position to persons with disabilities, preferably in a residential setting. Must have a valid NYS Driver License with an acceptable driving record.

This is a Full-Time, Non-Exempt Position.
