Neighborhood Health Center (NHC) is GROWING and actively seeking a Grants Manager. The Grants Manager will have responsibilities that include, but are not limited to, preparing grant reports and renewal applications, coordinating grant project proposals, and supporting all aspects of fundraising. For more than 30 years, NHC has delivered quality healthcare to the communities we serve.

Responsibilities:
• Shares responsibility for day and evening manager on duty (MOD) at the sites and assures the daily huddles are completed.
• Reviews and develops a thorough understanding of all current grant contracts and programs.
• Assures alignment of grant portfolio with strategic plan and emerging community needs. Aligns reporting with the format of the strategic plan goals.
• Assure an implementation plan is in place for each open grant, at times assists in the creation of the implementation plan with goals and deadlines.
• Holds grant work groups accountable for deadlines and grant compliance requirements and establishes grant outcome reporting measures, grant documentation, develops dashboards for those grants when appropriate.
• Assists in writing grants for government and foundation funders.
• Prepares and manages timely completion of grant reports and renewal applications. Keeps and follows reporting and renewal calendar, prepares reports and renewals for submission and obtains necessary information from managers as required.
• Identifies potential funders for NHC programs and capital needs. Reviews application guidelines to determine if foundation, government agency or corporation is a good match for NHC goals.
• Able to complete and submit certificate of need submissions to department of health through NYSE-CON system
• Assembles proposals and LOIs for submission including all required documentation and packaging. Maintains quality control over grant submittals.
• Assists CEO in working with partner organizations to assess and develop community service programs to meet the evolving needs of patients and the community. Maintains and develops community partnerships to meet the needs of patients and emerging community needs.
• Works with CEO, CFO and management team to develop budgets for grant proposals and applications.
• Meets regularly with the CFO to coordinate and update details of grant project budget utilization and reporting
• Works with the CFO to create a detailed report monthly on grant funds. Uses that data as parted of accurate financial reports for managers, CEO and funders.
• Develops professional working relationships with private and public funder contacts.
• Develops and maintains a current roster with contact information of potential grants, application deadlines, deliverables/goals, invoices and report deadlines.
• Assists CEO in working with partner organizations to assess and develop community service programs to meet the evolving needs of patients and the community. Maintains and develops community partnerships to meet the needs of patients and emerging community needs.
• Keeps CEO informed of grant application and reporting activity, awards and declines. Submits monthly internal reports of activity.
• Works with the CEO and leadership team to select and train managers and staff to meet the deliverables of grants.
• Supervises and hold accountable staff to ensure the coordination of services related to grants
• Utilizes our consistent marketing and messaging to ensure grant requests and proposals communicate NHC mission and goals.
• Uses needs assessment data in conjunction with partnership lists to assess strengths and gaps in partnerships.
• Assists in data collection and reporting for organizational purposes. Utilizes UDS mapper and other GIS data to determine area demographics to support grant applications and outreach events.
• Seeks out and presents innovative opportunities for CEO, leadership team and board to consider.
• Participate in the creation and implementation of a targeted community relations plan to identify partners that managers will engage with to build community capacity and relationships.
• Develops and submits a monthly report to the CEO on accomplishments in key areas of all grants in progress and other pertinent work.
• Manages development strategies for NHC
• Attends Board of Directors meetings as needed to report on the status of projects.
• Cultivates prospective donors and supporters, development of a major gifts program and other related initiatives.
• Key in fundraising committee to support all aspects of fundraising including the annual event
• Assists in developing and implementing procedures and policies and metrics to measure effectiveness of this position.

Requirements:
• BA college degree required
• Master’s in Public Health or other similar focus with healthcare experience preferred
• At least three to five years of health care related grant writing and development experience
• Exceptional attention to detail and follow-through

• Outstanding in creating PowerPoint presentations
• Proficiency in Adobe and database management
• Excellent oral and written communication skills
• Strong ability to diplomatically and effectively manage interactions with individuals at all levels and job functions, both inside and outside the organization
• Excellent judgment and discretion
