The primary role of the Care Management Coordinator is to coordinate care for our members in the community setting and assist in addressing social determinants of health (SDoH) across our Care Management programs.
The Care Management Coordinator gathers necessary information to develop an individualized care plan to address the member’s needs, health goals and care coordination.
Requires outstanding communication, comfortable working in team environment and exceptional telephonic customer services skills.
Full-time position Monday through Friday. No nights or weekends. Competitive salary and benefit package including health, dental, life, and 401k match.

Salary: $19.00 - $20.00 per hour
