Care Coordinator in Buffalo, NY

Salary Range: $20-$24/hr.

Care Coordinator – Responsible for coordinating care and services for referred high risk patients diagnosed with depression to improve the quality outcomes, minimize the number of high-risk events, and reduce inappropriate service utilization. The Care Coordinator educates patients about depression and its treatment, provides behavioral activation, and monitors depressive symptoms and response to medication and/or psychotherapy using a structured instrument (e.g. the Patient Health Questionnaire- PHQ-9); works closely with the primary care provider and consulting behavior health provider to establish and revise treatment plans when patients are not improving; and offers a brief course of counseling for depression (e.g., Problem Solving Treatment in Primary Care (PST-PC). Hourly Rate: $20-$24.
