The Supportive Housing Program Outreach Housing Specialist provides support services to individuals living with a mental illness residing in an apartment-based program. Responsible for advocacy, linkage to external support, assisting with move-ins, and assisting individuals in becoming acclimated to their new living environment. Helps individuals in developing or enhancing skills necessary to live successfully in the community, including awareness of rights, self-advocacy, verbal negotiation, problem-solving, money management, housekeeping, personal hygiene, community mobility, shopping, nutrition management, and leisure time activities.
Minimum requirements: High School Diploma and five (5) years of full-time work experience working in the Human Service Field. Candidates who do not have the preferred educational qualifications will be considered based on significant experience with the population. Plus, a minimum of 1 year’s paid experience in a direct service position to persons with disabilities, preferably in a residential setting. Must have valid NYS Driver's License with acceptable driving record.
Full-time position, Non-Exempt position.
