Social Worker in Lockport, NY

Salary Range: $58,000 yearly

Mount View Assisted Living is seeking a compassionate and dedicated Social Worker to support the emotional, social, and psychosocial well-being of our residents.
This role plays a vital part in enhancing residents’ quality of life, assisting with care planning, family communication, and connecting residents with essential services and resources.

Schedule: Full-Time; $58,000/yearly

Key Responsibilities:
- Provide social, emotional, and psychosocial assessments of residents
- Develop and implement individualized care plans in collaboration with the healthcare team
- Serve as a liaison between residents, families, and staff
- Assist with admissions, transitions, and discharge planning
- Connect residents and families with community resources and support services
- Provide counseling and crisis intervention when needed
- Maintain accurate documentation and resident records

Qualifications:
- Bachelor’s or Master’s degree in Social Work required
- Current New York State Social Work license preferred
- Prior experience in long-term care, healthcare, or assisted living preferred
- Strong communication, organizational, and interpersonal skills
- Compassionate, patient-focused approach
