Linkage Navigator in Cheektowaga, NY

Salary Range: $21.30-26.62/hour

The Linkage Navigator operates in a unique capacity to support the whole-person health model of service delivery implemented at Endeavor Health Services. The role fulfils a care coordination and case management role to support the person’s overall health and wellness. The Linkage Navigator will act as a liaison between the person served and their care team including but not limited to their clinician, peer, nurse, clinical pharmacist, and provider. The role will work to address access to care, social determinants of health, and gaps in care within this team-based approach.
