Health Homes Case Manager in Derby, NY

Salary Range: $46,500-$55,000 Annually

Help clients navigate complex health and social needs with compassion and purpose. Derby & Community-Based.

What You’ll Do:
• Complete client assessments
• Build person-centered care plans
• Coordinate medical, housing, financial, and behavioral supports
• Engage clients with high needs
• Document services to NYS Health Homes standards

What We’re Looking For:
• Bachelor’s in psychology, social work, or human services
• 2–3 years community-based experience
• Strong communication + problem-solving
• Knowledge of community resources
• Valid driver’s license + reliable transportation

Community-based position requiring travel to client homes and community locations. Valid NYS driver’s license and reliable transportation required. Bachelor’s degree in human services or related field preferred. Experience in case management, behavioral health, social services, or community support preferred.
