Salary Range: $32.00–$37.00 per hour

Make a difference in your community! Harmonia Collaborative Care is seeking a part-time Certified Dietitian to provide nutrition counseling and education to individuals with health-related social needs.

What You’ll Do:
• Provide individual or group nutrition counseling
• Educate clients on healthy eating and meal planning
• Collaborate with care team to support client wellness
• Document outcomes and maintain compliance with NYS 1115 Program

What We’re Looking For:
• NYS Registered Dietitian (RD/RDN) or RN Dietitian with CDN
• Degree in Dietetics, Nutrition, or related field
• 1+ year of relevant experience (Medicaid experience a plus)
• Strong communication skills and ability to work independently
