Case Manager for Emergency Men's shelter & Community Access Center

Amy’s Place of Hope is a Community Access Center on Buffalo’s East Side, welcoming women, men, and families. Rooted in the love of Christ, we provide access to health, mental health, and dental care, along with 96 emergency shelter beds for men. JOB SUMMARY:
The Case Manager works in an office setting to successfully engage those experiencing or at risk of chronic homelessness in the City of Buffalo assisting them with basic needs, including housing. The Case Manager will assist by placing referrals for additional wrap around services based on their need. The Case Manager will be responsible in following through to make sure referral were successfully completed. You must be able to work weekends, Holidays, and overnight shifts.

ESSENTIAL DUTIES:
- Engagement and Service Provision
- Maintains regular contact with street and shelter clients
- Assists clients with navigating the social services network to obtain and/or maintain benefits
- Performs regular outreach throughout the community including to soup kitchens, shelters, encampments, and community organizations
- Participates in weekend, night and/or early morning outreach with the outreach team as needed
- Distributes PPE and other emergency items as needed or requested
- Conducts and scores housing-related and other assessments
- Collaborates with local agencies and case workers to build positive working and referral relationships
- Greets and welcome guests, visitors, and community partners in a respectful, compassionate manner.
- Provide information about services, daily schedules, and program guidelines.
- Assist guests with accessing resources such as meals, showers, lockers, and case management referrals (when available).
- Act as a liaison between guests and staff, promoting dignity and respect for all individuals.
- Always monitor all guests, anticipate crisis situations when able, and help to maintain a calm environment.
- Record Keeping
- Complete guest intakes, including gathering demographic, background, and service needs information.
- Enter and maintain accurate data in organizational databases and case management systems.
- Ensure confidentiality and compliance with HIPAA and other privacy requirements.
- Complete daily logs, incident reports, and other required documentation in a timely manner.
- Support program operations through scheduling, guest check-ins, and other administrative tasks.

OTHER:
- Attends and participates in regular staff and community meetings including but not limited to Erie County Coalition for Services to the Homeless
- Attends seminars and workshops as identified
- Participates in agency wide special events such as holiday programming, Project Homeless Connect, fundraising events, etc. as requested
- Other duties as requested by supervisor which may include occasional evening or weekend responsibilities

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:
- Previous experience in Social Work or related field
- Established knowledge of existing human service networks, especially in the city of Buffalo and Erie County
- Strong verbal and written communication skills
- Strong attention to detail and follow-through
= Computer literacy
- Preferred experience with database utilization such as a Homeless Management Information System, Electronic Medical Record, etc.

QUALIFICATION STANDARDS:
- Associate degree in social work or related field with five plus years of related experience OR Bachelor degree in Social Work or related field with three-plus years of related experience.
- Must have own transportation and valid driver’s license

ERGONOMIC REQUIREMENTS:
- Sitting, standing, stooping, bending, standard lifting (50 pounds), carrying, some repetitive movements. All of the ergonomic requirements must be performed without significant risk of injury to oneself or others, or otherwise demonstrate or explain how one can perform essential functions listed above.

Job Type: Full-time

Pay: $50,000.00 - $55,000.00 per year
