Salary Range: $23.07-$27.67/hr.

Endeavor Health Services is seeking a Licensed Practical Nurse (LPN) to join our Critical Time Intervention (CTI) team. In this role, you will support individuals during key life transitions from institutional settings (such as hospitals or inpatient care) back into the community.

You will play a critical role in ensuring continuity of care, stability, and recovery, using a structured, time-limited intervention model focused on engagement, coordination, and long-term support connection.

Qualifications
• Active Licensed Practical Nurse (LPN) license (required)
• Valid driver’s license and reliable transportation
• Flexibility to work occasional evenings or weekends as needed
• Minimum 2 years of experience preferred in one or more of the following:
- Community health nursing
- Behavioral health
- Care coordination or case management
• Strong understanding of community systems, including:
- Medical and behavioral health services
- Housing and supportive housing programs
- Social service and benefit systems
• Proficiency with Microsoft Office applications

Pay : $23.07 – $27.67/hour based on experience, plus a $5,000 sign-on bonus*
