Salary Range: $88,000.00 - $94,000.00 per year
Pay: $88,000.00 - $94,000.00 per year$5,000 Sign-On Bonus
The Afterhours Triage RN is key to working with and providing assistance to the on call Advanced Practice Provider (APP).
The Afterhours Triage RN is an exempt position with the shift of continuous coverage each week from 5:00 pm Friday through 5:00 pm Sunday.
The Afterhours Triage RN must be flexible and available throughout their entire scheduled shift and be capable of multitasking between different priorities.
EDUCATION: RN licensed in New York State, Bachelor’s degree preferred
EXPERIENCE/ ABILITIES:
• 3 years RN experience preferred, acute care or emergency medicine preferred.
• 3 years Triage experience recommended.
• Excellent written, verbal, and listening communication abilities.
• Must be able to build and maintain collaborative, cooperative, and productive relationships both over the phone and face to face with all internal and external customers including members and their families, physicians, the health plan, vendors, and other health care providers as well as any other stakeholders in the community.
• Must be flexible and supportive of team members whether assigned work is telephonic or in the field.
• Computer skills necessary: email, electronic medical record management, Word, and Excel.
• Ability to meet and maintain necessary requirements per facility guidelines based on job assignment. These requirements may be outside the scope of Family Choice policies.
TASKS:
• Perform after-hours telephone triage, assessing member needs and prioritizing calls based on acuity.
• Return calls to facilities at provider’s request for non-acute matters, ensuring timely communication and issue resolution.
• May perform onsite assessments of patients.
• Follow- up on after hours sign outs by reviewing electronic health records to verify completion of labs and diagnostic testing.
• Contact facilities to confirm results, obtain reports, and communicate findings to the on-call APP to support clinical decision-making.
• May assist On-Call APP with lower acuity member concerns, including tele visits per APP request.
• Assist with member diversions from ALF to SAR or home to SAR.
• Follow up with acute members to monitor status changes when requested.
• Coordinate after hours sign-outs, requesting case owners complete detailed handoffs to support continuity of care.
• May triage CSNP afterhours calls and be available to see member in home.
• Other duties as assigned.
