Social Care Network Navigator: The Social Care Network Navigator performs the day-to-day activities of Endeavor’s participation in the Western New York and Finger Lakes Region Social Care Networks (SCNs) as a result of the NYS 1115 Waiver. The role is responsible for conducting screenings related to Social Determinants of Health (SDoH) and providing person-centered navigation support to connect community members to resources and services that address gaps in care. The Navigator will act under the direction of the Coordinator of Collaborative Care and will be responsible for ensuring the delivery of Social Care Network services provided by Endeavor. $20-$22/hour
