BHT (Behavioral Health Team) Case Manager to join our team to work with the NFTA. The BHT Case Manager functions as a specialized member of the treatment team and will provide direct service by linking clients to providers of services in the community as determined in the treatment plan, and will work closely with mental health counselors in the role of expediting, advocacy and monitoring to ensure that all services outlined in the client’s treatment and discharge plans are provided. $21.30-$24.00/hr.
