Salary Range: $18.70-$19.25/hr.

Agency Requirements:
• Valid NYS Driver's License
• Use of personal vehicle during working hours

Agency Expectations:
• Be available for supervisory consultation 24/7
• Demonstrates knowledge of program, mission, and services of Pinnacle Community Services, Inc., (PCS)
• Complies with all policies and procedures of PCS (including NASW Code of Ethics, Collective Bargaining Agreement, and Corporate Compliance Policies) and all governmental and legal requirements
• Attends and participates in appropriate Agency staff trainings and meetings
• Advocates for cultural diversity/sensitivity as appropriate
• Reports all suspected incidences of child abuse/neglect to NYS Hotline
• Experience and knowledge on the use of computers, cell phones, and a broad understanding of the Internet and e-mail in the workplace

Job Responsibilities:
• Engage youth in face-to-face contact within 10 days of referral
• Provide support services focused on educational/vocational services and/or caseworker/life skills as assigned
• Conduct initial and routine Casey Life Skill Assessments/Learning Plans, Observations, Skill Inventories, and Career Aptitude Tests. Complete pre-post assessment comparisons
• Responsible for data collections related to client and program outcomes
• Meet with Supervisor for scheduled supervision on a regular basis
• Work in conjunction with 3D program team members to achieve program outcomes and assist with client needs
• Develop a Learning Plan which focuses on the nine domains of youth development
• Determine what visual supports, communication supports, mobility supports will be needed in order to maximize independence
• Develop a comprehensive academic/vocational plan embedded with data collection systems and periodic review dates for progress monitoring
• Provide Agency and collaborative support services and education regarding safe and affordable housing services
• Provide services to enhance self-sufficiency and sustain meaningful employment that is fulfilling to the youth as well as meeting employer needs
• Working in partnership with all team members, including referral source, natural supports of the youth, community providers, etc. to develop a Plan for Youth Development incorporating tangible practice and skill utilization
• Engage community partners (i.e. Niagara County Public Schools, NCC, Job Training Programs, work sites, etc.) to minimize barriers and enhance youth participation and success
• Attend required Agency and professional development trainings
• Facilitate Life Skills Groups as scheduled

PHYSICAL/ENVIRONMENTAL DEMANDS:
• Ability to work in a crisis intervention setting
• Job performed in a modern office setting
• Ability to sit, stand and walk through the day
• Ability to push, pull, bend, life or carry up to 20 pounds throughout the course of the day related to office equipment such as p.c., journals, and binders
• The function of this position usually performed community engagement with youth and families throughout the day. Some amount of office work will be required, however a good amount of stooping, kneeling, bending, crouching, lifting, walking, carrying and other movements may be required. Tasks involving working on a computer keyboard, computer software applications, calculations, cutting, labeling and taping involve extensive wrist and hand movement. All individuals are required to be able to perform these movements without significant risk of injury of themselves or others.
