UM/Transition Clerk in Depew, NY

Salary Range: $20.50 - $21.00 per hour

The UM/Transition Clerk is responsible to follow Utilization/Transition policies and procedures when carrying out their job functions.

TASKS:
• Assists RN with requests for levels of service by compiling data, entering authorizations, and faxing authorizations to participating providers, vendors and nursing/adult home care facilities after determination by RN or Medical Director.
• Receive and process requests for non-emergent transportation by scheduling trips with approved vendor and faxing information to nursing/adult home care facilities.
• May assist with documentation of Transitions of Care.
• May be assigned specific responsibilities as per departmental need and workflow.
• Work within assigned Team along with APP, RN, Social Workers and assist with UM/transition related tasks.
• Distribute all authorization letters, emails and documentation that are related to UM/transitions per process and/or guidelines.
• May send transition POC to the receiving care setting.
• Serves as UM liaison for facilities within designated team.
• Provide assistance to all FCNY programs as requested by management.
• Other duties as assigned.
EDUCATION: Minimum High school diploma; Associate or Bachelor Degree preferred

EXPERIENCE:
• Minimum five (5) years working in a secretarial/administrative support role in a health care setting.
• Familiarity with health care terms and general office procedures and multiple computer systems.
• Excellent written and verbal communication skills.
• Working knowledge of word processing, spreadsheet, and presentation programs.
• Able to perform accurate basic mathematical calculations.
• Requires outstanding people and communication skills. Must be able to build and maintain collaborative, cooperative, and productive relationships with all internal and external customers including members and their families, nursing/adult home care facility staff, providers, the health plan and any other stake holders in the community.
