The Associate Director of Nursing Services for Subacute Care plans, develops, and organizes, the day-to-day operations of the Subacute Unit in conjunction with the appropriate personnel. This position coordinates all professional admission and discharge processing, determines the extent of health status of the patient at time of admission, meets with family and/or patient during the admission process to establish the role as the facility contact person, and serves as the liaison to outside physicians, including surgeons.
This person will report to the Director of Nursing daily to convey patient status and other matters related to the provision of interdisciplinary care process. They will also review the 24 hour nursing reports to ascertain if changes are necessary in the patient’s interdisciplinary care plan and facilitate changes as appropriate.
Thorough documentation of any required reports within the department are necessary for this position, for both patients and staff.
New York State Registered Professional Nurse licensure
Experience in long-term care facility preferred
Minimum of two years of nursing experience and at least one year of experience in a nursing leadership role; experience in geriatric nursing preferred
No findings or convictions of resident abuse, mistreatment and misappropriation of property
Exceptional time management and problem-solving skills
Able to interpret patient information and make critical decisions about necessary actions
Ability to supervise and lead; committed to assisting in the training and education of health care personnel (i.e., CNAs, LPNs, etc.)
Ability to supervise and work well with and show respect toward other health care personnel (i.e., CNAs, LPNs, physicians, etc.), residents and their family members
Strong written and verbal communication skills; able to follow written and verbal instructions
Caring and compassionate bedside manner
Respect for residents’ privacy and rights; display of genuine concern and empathy for the elderly
Able to perform tasks while upholding high standards of quality, cleanliness and kindness
