SUMMARY OF DUTIES
To act as a liaison between the agency and the work experience site. To teach, monitor and engage the people we support during work experiences.
ESSENTIAL FUNCTIONS AND JOB RESPONSIBILITIES
Develop employment goals and plans for the people in supported employment programs.
Support the people to meet goals with training and support.
Evaluate and update goals as needed.
Ensure program and documentation guidelines are met for all Supported Employment Programs.
Facilitate and support communications with and between the people we support, employers, co-workers and treatment team.
Support the people with transportation needs by supporting with mobility training or individual transport.
Responsible for being on time to every scheduled work shift and recording hours worked accurately via Paylocity or timeclock punches.
Physical agility and ability to react to emergency situations, including Strategies for Crisis Intervention and Prevention – Revised (SCIP-R) certification as per regulatory guidelines.
Certification in CPR and all other required certifications.
Responsible for ensuring that confidentiality is strictly maintained as required by law and Agency policy.
Responsible for securing and protecting personal and agency related data by adhering to the Agency’s computer security policies.
Follow all agency policies and procedures.
High School Diploma or GED.
One year of experience working with people with developmental disabilities preferred.
Valid New York State driver’s license.
Reliable vehicle and willingness to transport the people we support as situations require.
Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, grasping, bending, stooping, squatting, kneeling and climbing.
Ability to move, reposition, and lift and transfer individuals as required by prescribed program/plan.
