Rate: LEVEL 1 $16.00, LEVEL 2 $17.00, LEVEL 3 $17.50

A Health Home provides Care Management to Medicaid members that meet the Chronic Condition Eligibility Criteria. Medical, behavioral health, and social service needs are addressed in a comprehensive, integrated care planning process. The Care Manager oversees and provides access to all services needed to ensure children (3-21 years) receive everything necessary to stay healthy, out of the ER, and out of the hospital.
DUTIES / TASKS (not necessarily all inclusive)
• Work flexible schedules
• Comprehensive care management; Provide assessments
• Care coordination and health promotion; Administer CANS-NY (Child & Adolescent Needs & Strengths)
• Comprehensive transitional care
• Individual and family support
• Referral to community and social support services
• Model and practice sensitivity, patience, fair treatment, and acceptance of diversity in all interpersonal interactions
• Complete any reports or assignments on time

• Reduction in preventable hospitalizations and ER visits
• Access to community supports

• Member satisfaction
• Work independently and meet timelines; Operate in a multi-tasking environment; Able to rationally prioritize
• Maintain confidential and sensitive information; Maintain accurate records
• Set a positive example for other staff; Effectively communicate in both oral and written form
• Degree Human Services or related - LEVEL 1: Associates; LEVEL 2: Bachelor’s; LEVEL 3: Bachelor’s w/ bilingual
• Score70% or higher on CANS-NY annually (Child and Adolescent Needs and Strengths)
• Must be able to acceptably complete and pass all background checks and pre-hire requirements
• Must start with and maintain a clear criminal background and driving record
• Intermediate computer skills; Able to use MS Office, including Outlook with ease
• Familiarity with the local Native American community
• Must be able to work remotely and in-person / No barriers to commuting
• Must be able to transmit and receive information electronically without obstacle
• Must have own, reliable transportation carrying and maintaining a minimum of 100/300 auto liability coverage
• Must have a valid NYS Driver License
• Ability to lift at least 30lbs
• Interpersonal skills to work cooperatively and effectively with individuals, groups, staff, and a diverse population
