We’re expanding, come grow with us!
Advance your career and make a difference at JFS of Western New York.
If you thrive on helping people, are community driven, and are looking for change, one of our many opportunities could be what you are looking for:

Jewish Family Services of WNY is accepting applications for a full-time Care Coordinator in its Adult Health Homes division.

Do you have a passion for working with individuals to help ensure that they receive comprehensive care coordination?
Do you have a case management background and the ability to work to resolve problems that may prevent access to needed medical services and advocate on client's behalf?
Are you culturally sensitive in working with diverse populations?
If so, then this full-time position provides a unique and rewarding opportunity for you to make a difference in the lives of many.

In working with your clients, you'll be promoting client rights and access to social service, social security, mental health, and other support services. As part of the Care Manager Team, you'll manage all aspects of care for Health Home consumers. These responsibilities include tracking and arranging appointments, working in partnership with treatment providers, offering consumer advocacy and education, and coordinating other aspects of the consumer's community services. Equally important, however, are keeping accurate documentation, maintaining case notes, and completing (and monitoring!) all required forms. For this reason, you must not only have proficient communication skills but must also have excellent writing skills and attention to detail.

An Associate's degree in a relevant discipline is required; Bachelor's degree preferred
Experience in care coordination with community-based client services or discharge planning desirable
Ability to effectively provide case management services, responding supportively, and with persistence, to client families
Familiarity with community linkage resources helpful
Willingness to work within the community and make home visits
Experience with electronic medical records a plus

This position pays $19 per hour and offers great benefits including 4+ weeks accrued Paid Time Off (PTO), plus 13 holidays, excellent health insurance options (BCBS), 401k and much more. We are conveniently located off of Delaware Avenue with free parking. Since this position requires work in the community, you must have a reliable vehicle, a clean NYS driver's license and required liability insurance levels of 100/300K. If hired, you must produce evidence of having received the Covid 19 vaccination and undergo a DOH background check.
