Jewish Family Services of Western New York is currently accepting applications for the following positions due to our continuing growth:

In working with your clients, you'll be promoting client rights and access to social service, social security, mental health, and other support services. As part of the Care Manager Team, you'll manage all aspects of care for Health Home consumers.

These responsibilities include tracking and arranging appointments, working in partnership with treatment providers, offering consumer advocacy and education, and coordinating other aspects of the consumer's community services. Equally important, however, are keeping accurate documentation, maintaining case notes, and completing (and monitoring!) all required forms. For this reason, you must not only have proficient communication skills but must also have excellent writing skills and attention to detail.


A Bachelor's degree in a relevant discipline is required
At least two years experience prodiving direct services to persons with serious mental illness, developmental disabilities, or substance use disorders or linking persons with serious mental illness, developmental disabilities or substance use disorders to a broad range of services essential to successfully living in a community setting. Experience in care coordination with community-based client services or discharge planning desirable
Ability to effectively provide case management services, responding supportively, and with persistence, to client families
Familiarity with community linkage resources helpful
Willingness to work within the community and make home visits
Experience with electronic medical records a plus
Proficient verbal & written communication skills for interactions and for documentation
Familiarity with and appreciation of issues related to people with diverse cultures and languages
