Care Management RN in Depew, NY

The Care Management RN has a key role to conduct on site assessments of members who are followed in the care management program.
The RN will ensure the member is receiving the most efficient care possible in the most appropriate setting including the safe and effective transition of members across the care continuum and ensuring the member's needs are met.
They will maintain regular communication with the member and continually assess the members care needs.

Minimum 3 years' experience in or related to Care Management/Case Management/Discharge Planning is preferred. Must have valid and active RN license in the state of New York.

Salary: $70,000.00 - $78,000.00 per year

Experience:
